Steps

  1. Activate Phyrexian Altar by sacrificing Eternal Witness, adding {B}.
  2. When Eternal Witness dies, Kaya's Ghostform triggers, returning Eternal Witness from your graveyard to the battlefield.
  3. When Eternal Witness enters the battlefield, it triggers, returning Kaya's Ghostform from your graveyard to your hand.

Cheaper, tighter, alternate lines

Altar of Dementia replaces Phyrexian Altar if the gameplan is milling the table rather than draining it, and it costs zero per loop rather than one black. For the kill, Blood Artist and Zulaport Cutthroat are the cleanest drain outlets; Altar of the Brood mounts the same threat on an artifact that most lists already run.
