The cost is real: six mana and a legendary spell in hand, which means it only goes in decks built around legendary creatures or planeswalkers, not as a generic wipe. In those decks, most notably Shanid, Sleepers' Scourge, it's close to unconditional.

Commander is the natural home for Urza's Ruinous Blast — legendary commanders are universal, so the casting condition is almost never a real barrier, and exiling all nonlegendary permanents your opponents control is devastating at a four-player table. Outside Commander, the card sees essentially no play: Modern and Pioneer both demand cheaper interaction, and six mana for a conditional wipe is nowhere near competitive in those formats. Legacy and Vintage have access to far more efficient board control and don't reward the legendary condition.
