In Legacy and Vintage it's a five-mana artifact with no immediate board impact, which is simply too slow for those formats even if it's technically legal. Modern has the same problem: five mana for a creature that doesn't threaten the board on its own gets outpaced by everything in the format. Commander is where Psychosis Crawler earns its slot, specifically in draw-heavy blue builds where it functions as a damage engine rather than a beater.
