Momentum Breaker wipes the board of all tokens the moment it enters — no mana, no activation, just an immediate reset on any token-based army your opponents have built. The cost is that it does nothing to non-token permanents, so against the wrong table it's a dead card; against the right one, it's a one-sided wrath that costs next to nothing to include.

In Commander, Momentum Breaker earns its slot in any black deck that expects token strategies at the table — Braids, Arisen Nightmare lists in particular want it as redundant token hate that doubles as a sacrifice target for her own trigger. Outside of Commander, token-heavy strategies are common enough in Pioneer and Modern that this can show up in sideboards, though dedicated token hoser slots usually go to cheaper or more flexible answers. In Legacy and Vintage the card is legal but the competition is too stiff to matter. Standard is its most relevant constructed home right now, where token swarm decks are a consistent part of the metagame and Momentum Breaker offers a clean, cheap answer.
