Heartless Act kills any creature without counters outright, or strips three counters off one that has them — two modes of disruption on a two-mana instant. The caveat almost never matters in Commander, where counter-laden creatures are the exception; Archfiend of the Dross is about the worst-case scenario you'll realistically hit. Toshiro Umezawa turns every opponent's creature dying into a free flashback, making Heartless Act one of the format's best targets for his ability.

Heartless Act is a staple in Pioneer and Modern black shells that need reliable two-mana creature removal — the counter clause is nearly irrelevant in those formats outside of specific Hardened Scales or Infect matchups. In Standard it competes with whatever the current removal suite looks like, but unconditional instant-speed removal at two mana always earns a look. Commander is where Heartless Act finds its most comfortable home: the average game has few enough counter-laden permanents that you're casting the destroy half the vast majority of the time, and instant speed matters for leaving up interaction on opponents' turns. Legacy and Vintage have access to faster and more flexible options, so Heartless Act rarely makes the cut there. It's not legal in Pauper, which is the only format where it's simply off the table.
