Thoughtleech is a Legacy and Vintage curiosity — formats where blue is ubiquitous enough that the life gain is real, but where a one-mana enchantment that doesn't affect the board still struggles to earn a slot. In Commander, it's a niche sideboard-style inclusion: in a meta heavy with blue decks, Thoughtleech can pad your life total meaningfully over a long game, but most green Commander lists would rather spend that slot on a card that generates actual card advantage or acceleration. It's legal in Oathbreaker as well, where the same logic applies at a compressed table.
