The cost is a four-mana commander in three colors who does nothing until she attacks or you engineer additional mill triggers, but any graveyard-value shell more than covers that overhead, and Teval, the Balanced Scale decks proved that pairing her with a commander that rewards self-mill is enough to make her a near-staple.

Commander is where Sidisi, Brood Tyrant lives — she's powerful enough as a general to anchor entire archetypes and useful enough as a 99 inclusion to appear across thousands of Sultai and self-mill builds. In Legacy and Vintage she's legal but irrelevant; a four-mana 3/3 that mills three is too slow for formats where the game is often decided by turn two. Modern and Pioneer offer slightly more hospitable ground, but dedicated graveyard decks there have faster, more focused tools, and Sidisi competes poorly on rate. Oathbreaker is the one 60-card format where she could plausibly appear as a signature spell target, but she's not broken enough to warp that format either. The honest read is that she was designed for Commander, plays best in Commander, and you should not expect to see her anywhere else.
