Plague Sliver puts a 5/5 trampler on the board for five mana, then hands your opponents a free kill switch — they deal 3 to you during each of their upkeeps if you control it, and any one of them can sacrifice a Sliver to permanently remove the drawback from the table. It belongs only in decks that can exploit the self-damage or steal/donate the card immediately; elsewhere, it's a liability dressed as a threat.

In Commander, Plague Sliver is a build-around piece in Sliver-tribal lists that run Sliver Overlord or The First Sliver — the cumulative upkeep damage across four opponents is a real clock on your own life total, so it only earns its slot when you can tutor it onto an opponent's side or protect yourself from the drain. Legacy and Vintage have no interest in a five-mana 5/5 that punishes its controller when far faster threats exist. Modern is legal but similarly uninterested. Plague Sliver's real home is narrow Commander shells, specifically punisher and donation strategies, where the drawback becomes the entire point.
