Painful Quandary is a Commander card through and through — the multiplayer math is the whole point, and hitting three opponents with a recursive tax each turn is a categorically different proposition than the one-on-one drag it becomes in Legacy or Modern. In 60-card formats it's simply too slow at five mana and too easy to play around when you only need to manage one opponent's hand.
