Mindstab Thrull forces an opponent to discard three cards — a massive hand disruption effect — but flanking and a five-mana cost attached to a 1/1 body make the math hard to justify. Cheaper, unconditional discard spells exist in every black shell, and this one doesn't survive the comparison.

Mindstab Thrull is a Pauper-legal card, and that's the only format where it could theoretically see play — though even there, black has access to better discard at lower mana costs. In Commander, paying five mana for a 1/1 that dies to a stiff breeze is a losing proposition; Blightning, Cabal Condor, or Syphon Mind deliver comparable or superior discard for less. Legacy and Vintage have zero interest in this rate. Mindstab Thrull is mostly a collector's curiosity from an earlier era of Magic design.
