Jar of Eyeballs is a slow tutor that earns its payoff only in decks that kill their own creatures repeatedly — in those shells, it eventually lets you pick any card off the top of your library. Three mana to cast and two to activate is a steep investment for an effect that arrives several turns late, so it earns a slot only when the death-trigger count is high and consistent.

Jar of Eyeballs is legal in Commander, Legacy, Modern, Vintage, and Oathbreaker, but it sees virtually no play outside Commander. In Legacy and Vintage, the tutor bar is set by Demonic Tutor and Vampiric TutorJar of Eyeballs doesn't come close. Modern has enough redundancy that a slow, conditional artifact tutor never makes the cut. Commander is the one format where the card finds a home: aristocrats and sacrifice-heavy decks can load counters fast enough that the activation becomes a near-guaranteed topdeck of whatever you need.
