Hideous Visage gives your entire team intimidate until end of turn for three mana at sorcery speed — most creatures can't block, and your alpha strike connects. It's a one-shot combat trick stapled to a narrow condition, and outside dedicated go-wide black aggro, the slot is almost always better filled by something that generates lasting advantage.

Hideous Visage sees essentially no competitive play in Legacy, Vintage, or Modern — formats where three mana for a one-turn evasion buff is categorically too slow. Pauper is its most plausible home, specifically in mono-black token strategies where opponents' creatures are routinely non-black and a wide board can close the game in one swing. In Commander, Hideous Visage fits narrowly into token commanders like Endrek Sahr or Teysa Karlov that can flood the board with black creatures and need a finisher — but even there, Trumpet Blast effects with haste support are usually more reliable. The sorcery speed and single-turn window are the core limitation across every format.
