The catch is the five-mana price tag, which limits it to decks that can repeatedly trigger the life-loss condition; in those shells, particularly ones that abuse Kiki-Jiki, Mirror Breaker to copy it multiple times in a turn, the value compounds fast enough to justify the cost.

In Commander, Famished Foragers slots cleanly into any black-red shell that manufactures life loss as a resource — aristocrats, group slug, and token sacrifice decks all create the density of triggers needed to make five mana feel fair. Outside of Commander, Pauper is its most realistic home: the format's slower clock and abundance of cheap life-loss enablers let it pull real weight without competing against more efficient card advantage engines. Modern and Legacy have little interest — five mana for a conditional draw-and-ramp effect doesn't clear the bar in formats where that slot is occupied by game-ending threats.
