Cabal Therapy strips the most dangerous card from an opponent's hand for a single black mana — and the flashback lets you do it again for a creature sacrifice. The information problem (naming blind) is real, but any deck that generates tokens or recurs creatures turns that into a nonfactor.

Legacy is where Cabal Therapy earns its reputation: Storm and Reanimator shells cast it turn one off a fetch-and-crack, naming the disruption piece they already saw with a cantrip, then flash it back off a Dread Return target or Ichorid trigger for free. In Vintage the effect is similarly brutal but competes with Mental Misstep and harder lock pieces. Commander is the format where it's most context-dependent — hand disruption is diluted across four players, so Cabal Therapy is at its best when you can combo off the same turn you cast it, using a sacrifice outlet and a creature you were spending anyway to strip the one player holding countermagic.
