Vital Surge gains you life equal to the greatest toughness among creatures you control — relevant only if that number is large enough to matter, and in most decks it won't be. A two-mana sorcery that does nothing to the board is a hard sell when the same slot can accelerate, draw, or answer a threat.

In Commander, Vital Surge is a fringe inclusion at best — high-toughness decks like Doran, the Siege Tower or Arcades, the Strategist shells can spike the life total meaningfully, but even there the card rarely makes the cut because life gain without a payoff is just padding. In Pauper the bar is higher and the competition tighter; one-shot life gain at sorcery speed doesn't tempo-fit the format. Legacy and Vintage have no realistic home for it. The card is legal across most formats but wanted in almost none.
