In Commander, Sweettooth Witch is exclusively a lifegain-matters piece — hitting three opponents at once makes the drain trigger meaningfully better than it looks in 1v1. Outside of Commander, it sees essentially no play: four mana for a 3/3 that gains no immediate board presence is too slow for Modern, Pioneer, and Legacy, where games are decided before the draining adds up. Pauper is legal but the card doesn't see a home there either, since dedicated lifegain synergy decks in that format have tighter, cheaper options.
