Surging Dementia makes an opponent discard two cards for two mana — that's the whole pitch, and it's a fine rate. The catch is that it's a one-shot sorcery with no upside beyond the discard, which keeps it out of most competitive lists but earns a slot anywhere discard quantity matters more than recursion.

In Commander, Surging Dementia is a fringe inclusion: targeting a single opponent at a table of four dilutes its impact, and sorcery speed means the two cards are already gone before anyone can react. It finds a real home in dedicated discard decks — Tinybones, Trinket Thief or The Rack-style Oathbreaker builds — where piling discards onto one player is the explicit plan. In Pauper, the two-mana two-card discard rate is genuinely competitive at common, and it slots into mono-black discard shells without friction. Legacy and Vintage have access to far more efficient discard, so Surging Dementia doesn't see play there.
