The cost is your entire token army, which makes it a finisher on card advantage rather than a mid-game play; in Thromok the Insatiable decks that are already sacrificing creatures for one giant threat, that cost is often zero.

Thromok the Insatiable is built to consume its own token board anyway, so running Skullmulcher first — banking five or six cards before Thromok devours the rest — turns what would be a raw tempo play into a full refuel.

Commander is where Skullmulcher actually earns a slot — token strategies run wide, and drawing five or six cards off a single creature at instant speed is the kind of payoff that closes out games. In Legacy, Modern, or Vintage it's a five-mana sorcery-speed draw spell with a steep additional cost, which doesn't compete with what those formats can do for card advantage. Skullmulcher is a Commander card through and through, and it doesn't pretend otherwise.
