In Commander, Sangrophage is too narrow for generalist black lists — the life drain adds up across a long game and 40 life doesn't fully insulate you from a cost you're paying every single turn. It earns a slot in sacrifice-focused decks that can route the upkeep trigger into a resource rather than a punishment.
