Sanctum Guardian trades itself to redirect any source of damage to you instead — functionally a one-time, unconditional Fog that protects a planeswalker, a combo piece, or your face from a lethal hit. The three-mana 1/4 body is irrelevant; you're paying for the activated ability, and in the right shell that ability wins games.

Sanctum Guardian sees essentially zero competitive play in Legacy, Modern, or Vintage — formats where three mana for a situational one-time redirect can't compete with counterspells, fast interaction, or combo redundancy. Commander is where Sanctum Guardian actually has a home: protecting a combo player on the turn they go off, or keeping a key creature alive through a damage-based board wipe. It fits tightest in white creature-combo decks that need to survive one specific turn, though it's still a fringe inclusion even there.
