Sadistic Sacrament lets you surgically remove three specific cards from an opponent's library for three mana — or fifteen cards for seven, which is less surgical and more a lobotomy. The cost is that it does nothing to the board state and wins you no friends at the table.

Sadistic Sacrament lives almost entirely in Commander, where targeting one player's library is a meaningful threat and the kicker cost of seven mana is reachable in the late game. In Legacy and Vintage it's legal but irrelevant — those formats end too fast for a sorcery-speed library surgical to matter when Surgical Extraction and Extirpate exist at instant speed and far lower cost. Oathbreaker is the one fringe case where targeting a specific opponent's combo pieces could be relevant, but the format is niche enough that it rarely comes up in practice.
