In Commander, Pulling Teeth is a role-player in dedicated discard or connive builds — it's not a universal staple, but it over-performs with Marvo, Deep Operative at the helm. In Pauper, one-mana discard is genuinely competitive, and the conditional draw clause gives Pulling Teeth a small edge over vanilla discard at the same cost. Legacy and Vintage have access to strictly more efficient hand disruption, so it doesn't see play there. Pioneer and Standard players can't run it regardless.
