Phyrexian Vault converts surplus creatures into cards at the cost of three mana and a life each time — slow but repeatable, and repeatable draw on an artifact is what black sacrifice decks actually want. It's not a turn-one slam, but in any deck that generates tokens or fodder, it earns its slot.

Phyrexian Vault is legal in Commander, Legacy, Modern, Vintage, and Oathbreaker, but it only meaningfully shows up in Commander. In Legacy and Vintage, three mana for a slow draw engine is laughably behind the pace — those formats have Skullclamp and Yawgmoth's Will to worry about. Modern has enough efficient sacrifice payoffs that Vault still can't compete with cheaper draw options. Commander is where the math finally works: games go long, sacrifice synergies are abundant, and an artifact that turns a 1/1 token into a card on any turn you have nothing better to do is genuinely useful.
