Pay No Heed prevents all damage from a single source for one white mana — full stop, no conditions. It's the cheapest damage-prevention effect available, but stopping one source once is rarely enough in Commander, which keeps it out of most 99s.

Pay No Heed is weakest in Commander, where one source of damage rarely ends a game and the card generates no tempo or card advantage in a four-player pod. In Pauper and Legacy it has more of a case — white creature decks occasionally want a one-mana way to survive a combat trick or push through a blocked attacker — but even there it sits below Fog effects that cover the whole board. Modern and Pioneer have enough redundancy in protection spells that Pay No Heed competes poorly for slots.
