Millstone costs two mana to put on the board and two more every time you want to mill two cards — that rate is too slow to threaten a 60-card opponent and laughably slow against Commander's 100-card library. Dedicated mill strategies have better options at every point on the curve, and non-mill decks have no reason to run it.

In Commander, Millstone is functionally unplayable as a win condition — milling two cards per activation against 100-card libraries means you need dozens of activations before a single opponent is threatened. Legacy, Vintage, and Modern have enough degenerate mill tools that Millstone never sees a second glance; Glimpse the Unthinkable and Fractured Sanity do more in one card than Millstone does in a full game. Pioneer is the one format where mill is a fringe competitive archetype, but even there Millstone is outclassed by every purpose-built option at the same price point. Across every format it's legal in, Millstone is a nostalgic artifact, not a serious inclusion.
