Lacerate Flesh deals damage to a creature equal to the number of cards in your hand, then replaces itself — so at four cards in hand it's already a four-damage instant that draws a card for two mana. The ceiling is real, but the floor is embarrassing: with two cards in hand it's a two-damage cantrip, and any competent creature survives that.

In Commander, Lacerate Flesh lands in hand-size-matters builds — Reliquary Tower and Thought Vessel strategies that routinely hold eight or more cards make this a cheap, cantripping removal spell that punches above its mana cost. Outside of those shells it competes against unconditional removal options and mostly loses. In Pauper, where hand-size payoffs are rarer and removal is judged on efficiency alone, it's too inconsistent to earn a slot. Legacy, Vintage, and Modern have enough better options at the same cost that Lacerate Flesh is a non-starter in those formats.
