Healing Hands is fringe-playable only in Pauper, where the card pool is shallow enough that a cantripping lifegain spell occasionally earns a slot in slower, life-matters brews. In Commander, 40 life makes 4 life gain nearly meaningless unless you're running a dedicated Oloro, Ageless Ascetic or Vito, Thorn of the Dusk Rose shell that counts each lifegain trigger — and even then, Healing Hands competes poorly against Soul Warden effects that trigger repeatedly. In Legacy, Modern, and Pioneer it doesn't see play; the efficiency bar is simply too high for a three-mana sorcery that replaces itself and does nothing to the board.
