The catch is the triple-green cost, which makes it nearly uncastable in any deck that isn't mono-green, and even there you're spending a card slot on a combat trick with zero lasting impact.

In Commander, Fog Patch is a fringe include in dedicated Fog-loop strategies — the kind that use cards like Isochron Scepter or Narset, Parter of Veils to recur prevention effects — but the triple-green pip disqualifies it from most multicolor builds. Pauper is its most plausible home, where cheap combat tricks see real play and the card pool is shallow enough that a one-sided Fog has marginal relevance in aggressive green stompy lists. Legacy and Vintage have access to strictly better options at every point on the curve, so Fog Patch doesn't register there.
