Eradicate removes a creature permanently and strips every copy of it from all zones — graveyard recursion, hand reload, library shuffles, all of it gone. Four mana for unconditional black removal is fair; the exile-and-exile-all clause is the reason you run it.

Commander is where Eradicate earns its slot — singleton format means opponents rarely run more than one copy of any given threat, but the effect shines hardest against commanders that keep dying and re-entering the battlefield, since exiling them from the graveyard (and all other zones) can strand a general in the command zone or force a tax without recursion upside. In Legacy and Vintage, four mana at sorcery speed is too slow against the format's actual threats, and the exile-all clause rarely matters when most engines don't revolve around a single creature name. Eradicate is not legal in Pioneer, Standard, or Pauper, which makes Commander and Oathbreaker its real homes.
