It's a niche lifegain spell that earns its slot only in decks that specifically reward life totals gained, not a generic defensive tool.

In Commander, Blessed Reversal occupies a narrow role: it can swing a life total dramatically when a player is being targeted by a go-wide aggro deck or caught in a combat with tokens, but it doesn't advance your board and won't win you the game. Legacy and Vintage have it legal but would never touch it — those formats have no patience for three-mana reactive spells that don't draw a card or change the board state. Modern could theoretically house it in a dedicated lifegain shell, but better options exist at every point on the curve. Blessed Reversal is a Commander-or-bust card, and even there it needs a payoff engine to justify the slot.
