Ageless Entity enters as a 4/4 for five and immediately threatens to run away with the game in any shell that generates consistent life gain — every point gained stacks counters permanently. The main ceiling is that five mana for a vanilla 4/4 is a bad deal if you're not triggering it, and unlike Agatha's Soul Cauldron it doesn't do anything the turn it arrives.

Commander is where Ageless Entity belongs — life gain synergy commanders like Oloro, Ageless Ascetic or Dina, Soul Steeper create the repetitive triggers needed to turn it into a genuine threat, and a 20/20 trampler by turn eight is a realistic outcome in the right shell. In Legacy and Vintage it's legal but entirely unplayed; the format speed means a five-mana do-nothing-immediately creature never competes. Modern has faster, more consistent pay-offs for life gain synergy, so Ageless Entity sits on the fringe there too. Commander remains the only format where its growth ceiling actually gets reached.
